Aging and Facial Anatomy: A Guide for Women in Their 30s, 40s, 50s and 60s.

Aging is a natural process that affects all aspects of our bodies, including the face. To better understand how aging transforms our appearance, lets examine the key elements of facial anatomy and how they change over time.


1. SKIN CHANGES

In our 30s: Fine Lines, minor pigmentation issues, and reduced elasticity may start to become noticeable.

In our 40s: Wrinkles become more prominent, skin may become drier, and age spots / sun damage spots may start to emerge.

In our 50s: Skin becomes thinner, more fragile, and you may notice a lackluster, dull appearance to the skin.

In our 60s: Deep wrinkles, sagging skin, and increased dryness now become more noticeable.

Why does this start to happen with age? Collagen fibres create infrastructure for elastin and hyaluronic acid, which are responsible for the skin’s elasticity and hydration. As we get older, we start to break down collagen faster than we can produce it. This break down is intensified with things like UV (sun exposure), smoking, exposure to the elements, poor diet,etc. When we lose collagen, we being to lose the support which leads to wrinkles and sagging. Our skin barrier becomes impaired letting resulting in a decreased ability to maintain hydration within the ski leading to a dull, dry lacklustre complexion.


2. BONY CHANGES

In our 30s: Theunderlying bony structure of our skull remains relatively unchanged.

In our 40s: Bone loss may begin, leading to subtle changes in facial contours.

In our 50s and 60s: Further bone loss occurs, resulting in more visible changes to the faces shape and contours.

As we age, the forehead moves forehead while the cheek bones move back. As the cheeks descend we lose support around the eyes which begin to appear hollow and deep set and are often associated with looking tired. Our jaw becomes thinner which leads to a lack of definition which result in the jowly look we all hate.


3. FAT PAD CHANGES

In our 30s: Fat pads provide a youthful volume and lift to the face

In our 40s: Fat pads begin to diminish, leading to a loss of fullness in the cheeks and temples

In our 50s and 60s: Further fat loss causes more pronounced hollowing and sagging in the mid-face and under the eys.

As we age the fat pads in our upper face begin to trophy (waste away) while those in our lower face tend to become fuller. As deeper fat pads start to get smaller this reduces their support of the more superficial fat pads which we begin to notice as sagging and jowling.

Noticeable effects of fat pad atrophy can include:

  • Eyebrow and eyelid drooping
  • Wrinkles and lines become more obvious due to the loss of fat to plump them out
  • As volume is lost below the eyes, undereye “palpebral bags” can appear (which were previously hidden under the superficial fat pads)
  • The tip of the nose may drop
  • Nasolabial folds become deeper
  • Lips straighten or become downturned
  • Marionette lines become visible
  • Jowls begin to form and can lead to the neck appearing saggy


4. MUSCLE CHANGES

In our 30s: Facial muscles remain firm and well-toned.

In our 40s: Muscles begin to gradually begin to weaken, contributing to the formation of expression lines and wrinkles.

In our 50s and 60s: Further muscle atrophy occurs, leading to visible sagging and a loss of facial definition.

As we age, some facial muscles like our frontalis (forehead) get weaker resulting in our brows dropping while other muscles like our Depressor Anguli Oris that runs from the corner of our mouth to our jaw begin to contract pulling the corners of our mouth down leading to a sad expression.

Acne is a common skin condition that affects individuals of all ages, including adults.  While it is often associated with adolescence, adult female acne can be a frustrating and distressing problem.  In this blog, we will explore the causes and presentation of adult female acne, focusing on the distinction between inflamed and non-inflamed acne.  Additionally, we will also discuss various treatment options specifically related to skin therapies, that can help alleviate this condition.

 

 

Understanding Adult Female Acne

Adult female acne can arise from various factors, including homonal imbalances, stress, genetics, and underlying medical conditions.  Fluctuations in hormone levels, particularly during the menstrual cycle, pregnancy or menopause, can trigger acne breakouts by stimulating an increase in oil production in the skin.  Additionally, if there is a family history of acne, the likelihood of experiencing adult acne may be higher.

Presentation

Adult female acne typically manifests itself either as inflamed or non-inflamed.  Understanding the distinction between these two types is essential for determining the appropriate treatment approach.

    1. Inflamed Acne:  Inflamed acne, also known as inflamatory acne, is characterised by redness, swelling, and tenderness.  This type of acne occurs when the hair follicles become clogged with excess oil, dead skin cells, and bacteria, leading to inflammation and the formation of painful, pus-filled lesions such as papule, pustules, and nodules.
    2. Non-Inflamed Acne:  Non-Inflamed acne, also referred to as comedonal acne, presents as open or closed comedones.  open comedones, commonly known as blackheads, appear as small, dark spots on the skin’s surface due to the oxidation of sebum abd debris.  Closed comedones, or whieheads, are flesh-coloured or white bumps that remain closed, typically appearing just beneath the skin’s surface.

 

What is the cause?

There is no single cause for adult acne, but the most common triggers are:

  • Hormonal fluctuations
  • Stress
  • Gut function
  • Medication
  • Climatic conditions
  • Cosmetic ingredients that irritate the skin and clog pores
  • Hereditary disposition to acne

Stress and hormonal imbalances are the number one aggravating factor in adult acne.

 

Gut Function and how it can contribute to adult acne:

The link between our gut health, hormones and skin is complex, as each can influence and impact the other.  Oestrogen and progesterone impact how quickly food is digested and subsequently moved through the digestive tract.  The gut also influences how hormones are metabolised and eliminated by the body.  If you have a sluggish gut, that can trigger an hormonal inbalance.

Our liver’s function is to detoxify our blood and help regulate our hormones.  If our liver is overloaded by being impacted with a sluggish gut, it has no capacity left to help regulate hormones and therefore this can also contribute to hormone imbalances.

Our gut microbiome (the organisms living in our gut) or  condition’s such as leaky gut, can directly impact our skin by reducing our ability to assimilate and utilise key nutrients, vitamins and minerals that are so essential for our skin health.

Rentention Hyperkeratosis and it’s role in acne

If you have oily and acne-prone skin, your skin’s natural ability to slough dead skin cells isn’t going to be as effective as healthy skin.  This is called rentention hyperkaratosis is where the skin cells become ” sticky” and don’t shed properly.  The skin cells stick to the surface due to the lack of water and essential fatty acids in the skin.  This abnormal shedding of the skin leads to the formation of a microcomedone under the skin 8 weeks before the acne lesion is visible on the skins surface.

Our skin contains an enzyme called the statum Corneum Chymtryptic Enzyme, and it’s main job is to dissove the bonds that hold these skin cells together so that they can detach naturally.  This enzyme is unable to work effectively unless it has enough water in the skin to do so.

By repairing the barrier, (this is what keeps the water in), we will help the skin to retain water and with the addition of Essentail Fatty Acids, will create a waterproof layer on the surface which will also repair and maintain the skins barrier.

Sunlight contains ultraviolet (UV) radiation, which consists of different types of rays. The types of UV radiation you’re probably most familiar with are UVA and UVB rays. These rays can affect your skin in different ways.

Effects of UVA and UVB on the skin

What is the difference between UVA and UVB?

UVA rays penetrate farther into skin than UVB rays, steadily destroying key substances in skin that give it its firmness and elasticity. UVA rays are a leading cause of wrinkles and a cause of, or major contributor to, every type of skin cancer.

One more difference: UVA rays penetrate glass, while UVB rays do not. Unless the window you sit by at work or the windows in your car are specially treated to filter UVA radiation, your skin is being exposed to UVA rays, making sunscreen an absolute necessity.

The SPF rating of sunscreen is related to protection from UVB rays. To ensure adequate protection from UVA and UVB rays, look for sunscreens labelled “broad spectrum,” which indicates they have been tested and are permitted to make that claim. With the Aspect Sun SPF50 range stocked in clinic, it is a very high protection mineral sunscreen cream that has been specially developed to provide broad spectrum UVA and UVB SPF 50+ from the sun.

To get maximum protection you should be reapplying your sunblock every two hours, covering up as much as possible and trying to avoid the hottest part of the day.

what is a tyrosinase inhibitor and why is it so important

Tyrosinase is the enzyme responsible for the first step in melanin production. It is part of a conversion process that creates the pigment (melanin) to be transported to the top layer of your skin resulting in a darkening of the skin. It is a defence mechanism against damage (inflammation, picking and UV rays to name a couple).

When cells have been damaged by the inflammation ie UV radiation, the Tyrosinase enzyme will send an excess of Tyrosine which will result in overproduction of pigment. This is what you see as pigmentation.

Research has established the best way to control an excess of melanin production is to target this crucial enzyme. The other reason we target this enzyme is it is only found in the melanocyte- meaning there are no side effects anywhere else when we inhibit it.

Tyrosinase Inhibitor products will reduce the enzyme Tyrosinase from over stimulating so there is no overproduction of pigment in the skin. We don’t want to stop this process from happening completely as it is there as a protective mechanism to stop harm to the skin, what we want to do is slow down the process and only have a melanin needed to protect the skin released.

The darker the pigmentation the longer it will take to fade, so if we can stop it from being darker from the start, then it is easier and quicker to fix. Tyrosinase Inhibitors will correct hyperpigmentation and reduce it from over producing but it does require continued use.

Our skin has a memory and once it’s been damaged hyperpigmentation can come back, especially if the cells are left unprotected. Tyrosinase Inhibitors are not only for those who have Hyperpigmentation, but also for clients that have healthy, even skin.

You might have a beautiful, even skin tone now, but we want to make sure it stays that way. Using a Tyrosinase Inhibitor will help protect your skin and prevent you from forming hyperpigmentation and oftentimes hyperpigmentation is lying underneath the skin and will surface later-on in life. You can prevent this hyperpigmentation from happening or from surfacing altogether.

For pigmentation an important ingredient to go with your tyrosinase inhibitor must be sunscreen as this in UV ray damage is the 1st step of protection.

Applying a sunblock with a minimum SPF30 will help protect your skin from UV damage including pigmentation

here are my product recommendations for protecting your skin

You must use a SPF30+ sunblock EVERY day, I will always recommend Aspect Sun SPF50. (As a runner I know this works). This will help protect your skin from the UV rays mentioned above.

It is a must for my clients that are treating pigmentation to be on a form of Tyrosinase inhibitor like Aspect Dr Complete Pigment Plus.

Try and avoid the heat of the day as melasma can be triggered not just by UV but also heat

A Retinol night cream such as Aspect Dr Exfol A is also recommended. A retinol helps to stimulate cellular turnover helping to bring damaged pigmented cells up to the surface to be removed by the natural desquamation process.
